- Visit our Campus
- Complete the Online Application or download an Application Form form to complete and return
- Request Teacher Recommendation Forms (Math & Reading)
- Print, sign and deliver Release of Records Form to current school
- Submit one of the following documents as Proof of Income:
a. Federal Income Tax Return (Form 1040 or 1040A)
b. Letter from Social Security Office stating amount of your Social Security or Supplemental Security Income (SSI)
c. Letter from the Department of Social Services indicating budget.
